Medical weight loss is not about suppressing appetite but changing how your brain and digestive system communicate about hunger. GLP-1 receptor agonists—a class of drugs that has revolutionized the treatment of obesity and type 2 diabetes.
At Apex Primary Care & Wellness Center, our Houston doctor helps patients know whether GLP-1 medications are appropriate for their health goals.
Here is what you need to know.
What Are GLP-1 Medications? GLP-1 (glucagon-like peptide-1) is a natural hormone your body produces after eating. This hormone is responsible for telling your brain you are full, signaling the pancreas to release insulin, and slowing down metabolism.
The GLP-1 receptor agonists are laboratory-made versions of this hormone mimics its effects and do so for long.
These medications work through:
- Central Effects: They target areas of the brain that control appetite.
- Peripheral Effects: They slow gastric emptying, meaning food stays in your stomach longer, extending that feeling of fullness.
- Pancreas: They enhance insulin release when blood sugar is high and reduce glucagon secretion, improving blood sugar control.
Weight Loss Results: What the Research Shows
The clinical trial data for GLP-1 medications are remarkable:
- Semaglutide (Wegovy): Patients lost an average of 6% to 16% of their total body weight compared to controls when combined with lifestyle changes.
- Tirzepatide (Mounjaro/Zepbound): Studies show weight loss of 18% to 20% of body weight, approaching the results of bariatric surgery.
- Oral semaglutide (Wegovy pill): The FDA approved the first oral GLP-1 pill for weight loss in late 2025.
- Orforglipron (Foundayo): Approved in April 2026, this newer oral option showed 12.4% weight loss and has no food or water restrictions (unlike the semaglutide pill, which must be taken 30 minutes before any food or drink).
Beyond Weight Loss: Cardiovascular and Other Benefits
These benefits have led the World Health Organization (WHO) to endorse GLP-1 receptor agonists as a long-term obesity treatment, recognizing obesity as “a chronic, relapsing disease that requires lifelong care”.
GLP-1 medications offer benefits that extend beyond the scale. Large clinical trials have shown they:
- Reduce cardiovascular risk by approximately 20% for major cardiac events (heart attack, stroke, cardiovascular death) in adults with cardiovascular disease.
- Slow progression to renal failure in high-risk individuals with type 2 diabetes.
- Improve blood pressure, cholesterol, and other cardiometabolic markers.
Important Considerations and Side Effects
While effective, GLP-1 medications are not without risks. Most side effects are mild to moderate and often improve over time as your body adjusts. Common side effects include:
- Nausea, vomiting, diarrhea, constipation
- Abdominal pain and indigestion
- Headache and fatigue
- “Eggy burps” (sulfur-like burps)
Other Serious but rare risks include: Gallbladder disease (gallstones, cholecystitis), Pancreatitis (inflammation of the pancreas), Medullary thyroid cancer (contraindicated in those with personal or family history of certain thyroid cancers), and Loss of muscle and bone mass.
Safety Warnings
There are critical safety considerations for GLP-1 users:
- Pregnancy and Contraception: The medications should not be taken during pregnancy. Women of childbearing age should switch to non-oral methods or add a barrier method for 4 weeks after starting or increasing the dose.
- Surgery: GLP-1 medications delay gastric emptying, which increases the risk of aspiration during surgery, including pre-operative fasting. Always disclose GLP-1 use to your surgeon and anesthesiologist before any planned procedure.
- Who Should NOT Use GLP-1s: These medications are contraindicated in individuals with: Personal or family history of medullary thyroid carcinoma (MTC), Multiple endocrine neoplasia syndrome type 2 (MEN2), and a history of pancreatitis.
Is Medical Weight Loss Right for You?
GLP-1 medications are powerful, but they are not quick fixes. The WHO emphasizes that these drugs “do not replace the need for a healthy diet and physical activity”. They work best as part of a medical weight loss program that includes nutrition counseling, lifestyle changes, and ongoing medical supervision.
At Apex Primary Care & Wellness Center, our Houston doctor will offer personalized primary care for weight management. We evaluate your health history, discuss your goals, and help you understand whether GLP-1 medications are appropriate.
